Medical Records Request

AEC sends a copy of your medical records to your regular veterinarian as soon as possible after an emergency visit.

If you or your insurance company or other speciality provider(s) also require a copy of the medical record, please explain in the message box below and carefully enter the desired destination email address in the "send records to" field below. Include additional addresses in the Message comment box if necessary.

After we receive your request a confirmation email will be sent to your email address. You will receive another email when your records request has been completed (normally within 1-3 business days).
